Good Faith Estimate

Under the federal No Surprises Act (effective January 1, 2022), you have the right to receive a Good Faith Estimate of expected costs before your care begins.

What is a good faith estimate?

You have the right to receive this estimate in writing at least 1 business day before your appointment (or at least 3 business days for services scheduled 3 or more days out), and any time you request one.

What can I do if I am billed more than my good faith estimate?

If you are billed for an amount that is $400 or more above your Good Faith Estimate, you have the right to dispute the bill.

Keep a copy of your Good Faith Estimate.

How can I request my health records?

You may request your records by emailing clpad@clearlakepsychotherapy.com with your full name and date of birth.

Please allow up to 15 business days for processing. A fee may apply to copies.

Who licenses and regulates Clear Lake Psychotherapy's clinicians?

Clear Lake Psychotherapy's clinicians are licensed and regulated by the Texas Behavioral Health Executive Council (BHEC). If you have questions about a clinician's license or wish to report a concern, you may contact BHEC directly: bhec.texas.gov/contact-us

How do I file a consumer complaint?
